Refer a single adult who needs transitional, supported or temporary housing. This pathway is for individuals only — not families or multi-person households.

A short form covering name, age (18+), gender, contact details, benefits, current housing and what they are looking for.

Who can use this form

One adult per referral. A referral does not guarantee a placement.

Professionals

HSPs, local authority teams, charities, outreach, social workers and other support professionals referring an individual into housing.

Individuals

Single adults who need transitional or supported housing, including self-referrals.

Not for families

What we ask for

Only the essentials so we can understand the individual’s needs quickly.

Name & contact

The individual’s full name, age (18+), gender, email and phone number.

Benefits

The applicant must have Universal Credit (UC) plus at least one other benefit from the list.

Housing & preference

Their current housing situation, whether they want a studio / 1-bed flat or a house share, and a preferred move-in date if accepted.

Referrals include personal data about one person. We use it only to assess housing suitability and to communicate about the referral.
